USY LOCK-IN

February 22-23, 2025
Meet at Adath at 7pm on Saturday, February 22, for Havdalah.
We will then take a bus to Urban Air, 8pm until 10pm. 
We will return to Adath for the overnight lock-in.
The Lock-in concludes Sunday, February 23, at 8:30AM.

$25 per person includes the bus to and from Urban Air*, admission, and snacks.
*Urban Air requires a parent/guardian fill out a paper waver, which will be available for collection and completion.

My child/ren has/have my permission to attend the USY Lock-in and activities February 22-23, 2025. I agree to indemnify and hold harmless Adath Jeshurun Congregation (reference to Adath Jeshurun includes their employees, Board members, volunteers, and members) for any damage that my son/daughter may cause during the event. I release and waive any claims against Adath Jeshurun that my child may have for any injury that may occur in transportation or related to the event.  I give permission to Adath Jeshurun’s representative to authorize medical treatment or other measures, which must be taken for the good of my son/daughter in the event I cannot be reached in an emergency.
If this waver is not filled out, there is not participation in the Urban Air event.
During the activity, I may be reached at the following phone number.
If I cannot be reached in the event of an emergency, the following person is authorized to act in my behalf.  
$25 per child covers the bus, admission to Urban Air, and snacks.
