Everyone is welcome at Adath — there’s no fee to attend services. If you feel moved to give this High Holy Days season, we invite you to make a donation through the L'Chaim Campaign.

Name tags for High Holy Days will be created using the information submitted in this form. Please record only one (1) First Name and one (1) Last Name in each slot exactly how it should appear on a name tag. Children under 5 do not need to be listed.
